Case Manager 

SUMMARY: LifeSpan Resources is the designated Area Agency on Aging and Aging and Disability Resource Center (ADRC) for Clark, Floyd, Harrison and Scott counties. The Case Manager will fulfill the agency’s function of being the intake point for the Indiana Division of Aging’s Home and Community-based Service programs. This would also include other agency collaborations to include, private pay initiatives, federal programs and care transition activities. The goal is to accurately, consistently, and efficiently provide access to quality home and community based services.

SPECIFICALLY: The Case Manager is responsible for educating, advocating and working with individuals of all ages, their caregivers and representatives to create person-centered plans. Screening and assessment tools are used to discover individual’s strengths, values, preferences and eligibility as required by various programs with the goal of maintaining independence in the community. Case Managers may work in a variety of settings that include in-office, client home settings, hospitals and skilled nursing facilities.

Skills/Abilities

Excellent communication skills, both oral and written, with good interviewing skills. The ability to assess specific needs both in person and on telephone. The ability to empathize and understand individual behaviors. Knowledge of options, funding sources, services, resources, and information available in the community.

Education Requirements

A Bachelor’s Degree in social work, sociology, gerontology, psychology, counseling, or nursing; OR

A registered nurse currently licensed to practice in Indiana with one-year experience in a human service setting; OR

Bachelor’s Degree in any other field with a minimum of two years full-time, direct service experience with the elderly or persons with disabilities. This experience must include assessment, care plan development and monitoring; OR

Master’s Degree in a related field may substitute for the required experience.

Nutrition/Wellness Coordinator/Driver at Bacala Senior Center in Scottsburg, IN

Duties include: manage the Title IIIC Elderly Nutrition Program, organize Health and wellness programming for the meal site at the Center and provide transportation to and from the meal site for attendees.   Successful candidate shall possess the following:  detail-oriented, highly dependable, sensitive to needs of the elderly, work well with people of all ages, outgoing with high energy level. Possess or willing to accrue a For Hire Certification  Driver’s License (written test only), high school graduate or higher, and some computer skills (email, fax, MS Word, etc). Background in health education, social services, physical/recreational therapy or similar experience helpful.  Lifting up to 25 lbs and standing, auto insurance, reliable transportation, and drug testing, driving record check and background check.

Hours: 40 hrs per week. Mon-Fri. 8:30 a.m. – 1:30 p.m.
