Transportation

LifeSpan Resources furnishes transportation to life sustaining therapies (renal dialysis, chemotherapy, and radiation treatments), doctors, social service organizations, grocery stores, pharmacies, senior centers and other life enhancing locations. The LifeSpan Resources Transportation Team continually strives to accommodate all requests for transportation. We strive to be as dependable and efficient as we can. Please review the guidelines below to help us meet your transportation needs in a safe and reliable manner.

 

Scheduling Rides

 

Non Medicaid, Social Transportation and Private Pay:

Because our appointment times fill up quickly, please call us as soon as possible to schedule a ride. Calling several days in advance typically ensures we will be able to meet the requested appointment time. We are able to schedule appointments eight (8) weeks in advance. We will make every effort to meet transportation needs with short notice or to suggest an alternative solution. During the phone call to our Dispatcher, we will ask important information each time, including name, address, telephone number, destination address and special needs. All information is kept confidential. At times, our Dispatchers are away from the phone, so please leave a message on our answering machine and someone will return the call as soon as possible.

 

Medicaid:

If you are a Medicaid patient and need to schedule non-emergency medical transportation, please be advised:

 

The State of Indiana Family and Social Services Administration (FSSA), has contracted Medicaid medical transportation to Southeastrans.  This change is effective JUNE 1, 2018.

 

LIFESPAN WILL STILL PROVIDE YOUR TRANSPORTATION, HOWEVER:

 

You will need to call Southeastrans to schedule medical transportation trips. PLEASE REQUEST LIFESPAN RESOURCES AS THE COMPANY WHO WILL TRANSPORT YOU TO YOUR APPOINTMENTS.

 

Southeastrans will send the trips to LifeSpan Resources. The call center is now open to schedule your appointments.

Managed Care Medicaid clients must make reservations for non-emergency medical trips through LPC by calling 1-800-508-7230.

 

Continue to call LifeSpan Resources for social trips to the grocery, shopping, pharmacy, Senior Games, etc.  If you are a new rider, our schedulers direct you to our registration personnel (Options Counselors).

Pick Ups and Returns:

To make sure we get individuals to appointments on time, please be ready at least 15 minutes in advance of the scheduled pick-up time.  Due to weather, road conditions and traffic jams, the pick-up time could change.  If we find that the driver will be later than 15 minutes past the pick-up time, the Dispatcher will call.  Please feel free to call us and check on scheduled pick-up times.

 

Cancellation & “No-Show” Policy

If it is necessary to cancel a scheduled ride, it is very important to let LifeSpan or Southeastrans (Medicaid patients) know in advance. We would like at least two (2) hours notice prior to the scheduled pick-up time.  If the van driver is at the pick-up address and the individual the ride is scheduled for is not, this is considered a “no-show.”  Attention to this policy will help us to run our transportation service more efficiently.

 

Safety & Comfort

Indiana State Law requires that all passengers in any vehicle must wear a seat belt.  To ensure each individuals safety, as well as the safety of other passengers, please wear a seat belt. Drivers will assist with the seat belt, if necessary.  Eating, Smoking and Drinking by passengers are prohibited on any LifeSpan Transportation vehicle.  Abusive conduct by a passenger towards the driver or another passenger may result in the termination of transportation services. Abusive conduct includes any acts that are considered offensive and/or touching another person in a rude, insolent or aggressive manner. Verbal abuse in the form of swearing or hand gestures is also considered abusive conduct.

 

Transportation of Children

We are able to transport children to medical appointments. They must be accompanied by a parent or guardian. Child restraints must be provided by the parent/guardian of the child.  Child restraints must be used to transport children from birth through age eight (8).  A child restraint system or seatbelt must be used to transport children from age nine (9) through age sixteen (16), in accordance with Indiana State Law.

 

Personal Care Attendant

Personal Care Attendants may accompany all passengers on LifeSpan vehicles.  Please let office personnel know you will have an attendant when scheduling your appointment.

 

Service Animals

Service Animals are allowed to accompany a passenger and space will be made available.  Please let office personnel know when you will have a service animal accompanying you, when you schedule your appointment.  Please make sure the animal is securely leashed.

 

Passenger Assistance

If assistance is needed to help you enter or exit the vehicle, just let the office personnel know when scheduling your appointment.  However, you may request assistance from the driver at any time during your trip.

 

Portable Oxygen

Passengers with portable oxygen are welcomed on LifeSpan vehicles.  The driver will assist you, if needed.  Please let the office personnel know you will have portable oxygen with you when scheduling your appointment and if you need assistance.

 

Wheelchair Accessibility

All of our vehicles are capable of transporting riders in wheelchairs.  All drivers are trained in safe wheelchair transport and capable of helping riders in wheelchairs to have a safe and comfortable ride. However, our drivers cannot carry wheelchair passengers up or down steps. Anyone in a wheelchair wishing to use our transportation service should have a safe ramp at home for accessibility, or be ready outside on level ground where the driver can assist the rider into the van. All wheelchair passengers can have an aide ride with them for assistance.

 

Donations & Tipping

Donations are appreciated and can be given to the driver or mailed to our office at 317 E. Fifth Street, New Albany, IN 47150.  No one will be denied transportation service based on ability to donate.  Our drivers are not allowed to accept tips.  To express appreciation of the driver, please call us at the LifeSpan Transportation Office and let us know.
