POSITION SUMMARY
The Options Counselor will serve as the intake point for all long-term services and supports. The Options Counselor will meet with potential participants to determine initial service plans and levels of care for long-term services. They will work under close supervision of the ADRC Supervisor to accurately, consistently, and efficiently provide access to quality home and community-based services. This is a part-time position. Anticipated work schedule is not more than 20 hours per week.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Essential functions include but are not limited to the following:

  • Guide individuals and their care teams to make informed choices about long-term services and supports.
  • Be responsible for determining program eligibility by utilizing industry specific screening and assessment tools.
  • Develop initial service plans and determines levels of care for individuals.
  • Provide care transition services as established through private initiatives.
  • Manages the Medicaid application process.
  • Complete all steps of Waiver Intake, PASRR, MDS Section Q Referrals and Money Follows the Person programming within the specified business days as outlined in the ADRC Contract Scope of Work.
  • Participate in a minimum of 20 hours in meetings and trainings annually along with an IDA approved Case Management Orientation.
  • Act as a Subject Matter Expert in the resources available for program participants such as elderly or persons with disabilities.
  • Act as a liaison between the Agency and the State. Provide ready access of all documentation to the State as requested.
  • Comply with the Family and Social Services Association and IDA manuals, as well as Federal, State, and local laws.
  • Perform other duties as assigned by senior leadership.
  • Assist with screening, intake, orientation, referral, counseling, crisis response, patient education, case management, reporting and record keeping, and consultation with other professionals.
  • Educate, advocate, and counsel inquirers on available community and agency resources that will meet their needs. Carry out the functions of Options Counseling by responding to requests for information. 
  • Assess client needs, complete assessment screens and link callers to appropriate services.
  • Provide assistance to those in need by contacting the referral agencies and providing follow-up activities to ensure service access. 
  • Serve as the Agency’s single-entry point for Home and Community Based Services. Determine eligibility for home care and place on all applicable waiting lists. Oversee the application process for the Agency and maintain waiting lists for in-home and community-based services in Clark, Floyd, Harrison, and Scott counties. 
  • Provide warm hand-offs for crisis intervention services to those in need of immediate assistance. Link callers with emergency services and coordinate activities with other service entities. Provide follow-up services to assure needs are met.  
  • Assist in preparation of ADRC reports by entering/tracking data utilizing current MIS.
  • Conduct in-depth assessment of person’s needs, strengths, and preferences; collect all necessary needs assessment information and complete assessment tools. 
  • Develop and implement a Plan of Care to meet the individual’s needs with the client and/or responsible party. Review and explain to client/guardian and have that individual sign to show understanding. 
  • Maintain and document in a narrative each client contact and monitor the quality, utilization, and cost effectiveness of services as the program requires. 
  • Conduct annual reassessment and updates of eligibility in a timely manner and in accordance with program requirements, and to avoid gaps in service authorizations. 
  • Facilitate and monitor formal and informal supports to maintain consumer safety and feasibility in the community. 
  • Assist with training and orientation of new Care Managers and provide case consultation as needed.
  • Employ a method of checking, regulating quality of care, services, products and determining goal achievement of plan of care.
